本义是指本来已经错了，又犯了新的错误。现在也用来形容事情一件接一件地出错，坏事接连发生。

Origin Story
小明参加了学校的演讲比赛，他精心准备了演讲稿，但由于紧张，在台上结巴不断，演讲效果不佳。演讲结束后，小明很沮丧，决定下次再接再厉。然而，回家后，他又不小心把妈妈新买的瓷器打碎了，这让他更加懊恼。本来演讲就失败了，现在又闯了祸，真是错上加错。
